Why didn’t my insurance company pay this bill?

If your insurance company denied this bill, or if they did not pay the amount you were expecting, please call your insurance company. They can explain your coverage terms and limits and why they may have denied your bill. The phone number for your insurance company should be printed on the back of your insurance card.

Why am I getting a bill from this provider?

This bill may be for services provided by a physician in the hospital or facility where you received treatment. Alternatively, this bill may be for medical services (biopsy tissue processing, examination and interpretation) from a facility where your treating doctor has referred a tissue sample for examination. Please note, you may also receive a separate bill from the hospital or medical facility for additional services received.

What is your return/refund policy?

If you dispute a charge or feel you are due a refund, please contact our customer service department. Please allow 4 to 6 weeks for your account to be reviewed.
